化脓性腮腺炎与流腮均表现为发热及腮腺肿大,故临床上要注意两病的鉴别,其鉴别要点简要介绍于下。
病原

发病季节及年龄

腮腺局部情况
化脓性腮腺炎多为单侧。少数为双侧。其腮腺肿大速度不似流腮那么快、而且与感染轻重程度呈平行,腮腺早期质较硬,局部疼痛及触痛较明显,局部皮肤发红、发热,晚期肿大腮腺可有波动感。流腮腮腺质韧,可有轻压痛,局部皮肤不红。
腮腺管口
两病腮腺管口均可有红肿,而以化脓性腮腺炎为明显。压迫腮腺时。化脓性腮腺炎可见腮腺管口有脓液溢出。
周围血象
化脓性腮腺炎可见白细胞总数增高,分类个性核白细胞比例明显增高。
细菌培养
化脓性腮腺炎时腮腺管口引流培养,可培养出致病菌。
